Lyla Care Protection is designed to provide coverage for your custom Lyla Retainer, Nightguard, or Sports Mouthguard—so you can stay consistent with your routine without the stress of unexpected replacement costs. Our goal is simple: protect your investment and keep you supported, even when life happens.

Lyla Insurance Policy

Coverage:

This insurance policy provides coverage for select products purchased from Lyla—one product for $9.99/month or two products for $14.99/month. This ongoing subscription allows you to purchase a replacement device for only $50 in the event of loss, damage, or if you simply need a backup. As long as your subscription remains active, you’re protected—no questions asked—under the following conditions: Revived Smiles

Damage Coverage:

• 
  Improper Use & Negligence: Damage resulting from improper use or negligence. Revived Smiles
  
• 
  Loss or Theft: Coverage for lost or stolen items. Revived Smiles
  
• 
  Accidental Breakage: Protection against accidental breakage. Revived Smiles
  
• 
  Natural Disasters & External Factors: Coverage for damage resulting from natural disasters, fire, water, power surges, or other uncontrollable external factors. Revived Smiles
  
• 
  Cosmetic Damage: Cosmetic damage that affects the functionality of the product. Revived Smiles
  

Exclusions:

• 
  Normal Wear and Tear: Excludes coverage for normal wear and tear. Revived Smiles
  

Service and Claim Processing:

• 
  Discretion of Service: The determination of the appropriate service option will be at the discretion of Lyla, based on the specific circumstances of each claim. Revived Smiles
  
• 
  Inspection Requirement: Customers may be required to ship the product back for inspection. Revived Smiles
  

Eligible Products (Lyla):

This plan covers only the following products:

  Retainers Revived Smiles
  
• 
  Nightguards Revived Smiles
  
• 
  Sports Mouthguards Revived Smiles
  

Replacement Policy:

Lyla will replace the insured item in accordance with the guidelines below, as long as the subscription is active and subject to the conditions and exclusions outlined above:

• 
  Custom Retainers, Nightguards, and Sports Mouthguards are eligible for replacement twice per year (once every six months). Revived Smiles
  
• 
  A $50 recrafting fee is required at the time of each replacement claim. Revived Smiles
  
• 
  Obligation Satisfaction: If the insurance is utilized for any one product, the insurance obligation for that order is considered fulfilled. Revived Smiles
  

Return and Refund Policy:

• 
  Refund Approval: In the event your appliance is refunded, the insurance will also be refunded. Revived Smiles
  
• 
  Non-refundable and Non-transferable: Under no circumstance (other than the one mentioned above) is the insurance refundable or transferable. Revived Smiles
